My comp. is $6,000 plus childcare, mileage, co-pays, etc. IP's will also be responsible for all travel/hotel accommodations & a $50 a day food allotment should I be required to travel for any purpose. I may also request to have a companion accompany me which would also be at the expense of the IP's. IP's or their coordinator will be responsible for scheduling all appointments, travel arrangements, paying for all appointments, etc. I will ask for $750 at start of meds with . paid the day of retrieval. Childcare will need to be paid in advance also but only as advanced as the day prior to having to travel. All my fee's are those of which anyone would request.
